Who We Serve:
Transitional Case Managers (TCM) provide support to individuals 16+ whom are experiencing mental health concerns that significantly interfere with a person’s ability to function in areas such as daily living, housing, finances, employment, education, relationships, leisure and health. An individual and their TCM will work together to create and implement a plan for enhancing stability and identifying and meeting immediate needs and short term goals.
What We Do:
TCM’s work with an individual for up to 4 months to help them navigate community supports, connect to resources and services and meet short term goals.
Some of the things a TCM can assist someone with include:
- Basic needs (housing, income, budgeting, activities of daily living)
- Strategies & education to enhance mental health and wellness
- Increased independence (eg. Transportation, self-advocacy)
- Crisis Management/Prevention
- Support with concerns related to work/school
- Support with concerns related to mental illness and/or addiction
- Strengthening interpersonal relationships and connecting to supports and services that can assist you now and in the future
We also offer a service called Transitional Case Management Outreach. This service is a partnership among CMHA Middlesex, London Health Sciences Centre and Mission Services of London. The goal of Transitional Case Management is to support individuals who have been hospitalized to achieve a smooth transition from hospital to home. Before hospital discharge you will be linked with appropriate mental health services in the community. For approximately 9 weeks, you will be supported by the Transitional Case Manager to ensure your immediate needs are being met after your hospitalization. This warm transfer improves your health outcomes and stabilization back to community. Most of the referrals for this program come from the mental health inpatient floor, Centralized Emergency Psychiatric Services and the Emergency Department at London Health Sciences Centre.

Transitional Case Managers are available Monday – Friday from 8:00 a.m. – 4:00 p.m. at our Queens Avenue Site. The telephone number for Queens Ave. is 519-668-0624. An individual working with a TCM will arrange times to meet during these hours on site or within the community.
