“Living well” with a serious mental illness means different things to different people. Our Case Managers give you the one-on-one support and encouragement you need to live well.
At CMHA Middlesex, you can participate in Case Management if you have mental health needs that significantly interfere with your ability to function in areas such as daily living, housing, finances, employment, education, relationships, leisure and health. We also support individuals with concurrent disorders (a mental illness and an addiction). Your relationship with your Case Manager is a voluntary partnership where you identify areas of need, and together you and your Case Manager develop a plan for reaching your recovery goals.
Case Management Services are generally offered for 4 – 6 months. We encourage active participation in the recovery process throughout that time. If, in the future, a new need develops, you can call your Case Manager and rapidly re-enter with the program.
Case Managers are available Monday – Friday, 8:30am until 4:30pm. They are located at our Queen Street location, My Sisters’ Place, and Women’s Community House. Case Managers are also available to meet at various locations in the community as appropriate for you.
We also offer a service called Transitional Case Management Outreach. This service is a partnership among CMHA Middlesex, London Health Sciences Centre and Mission Services of London. The goal of Transitional Case Management is to support individuals who have been hospitalized to achieve a smooth transition from hospital to home. Before hospital discharge you will be linked with appropriate mental health services in the community. For approximately 9 weeks, you will be supported by the Transitional Case Manager to ensure your immediate needs are being met after your hospitalization. This warm transfer improves your health outcomes and stabilization back to community. Most of the referrals for this program come from the mental health inpatient floor, Centralized Emergency Psychiatric Services and the Emergency Department at London Health Sciences Centre.
