CMHA Middlesex takes your concerns and your complaints seriously. These complaints may be about the services you have received, or about an individual who you feel is causing you distress and may be a barrier to your recovery. When we hear your complaints, and investigate the issues you bring forward, it helps us to continually improve our services and our organization.
The first step is to speak directly to the individual or service you are unhappy with. You may want to invite the Manager of that department. If the problem is still not resolved, please complete the complaint form below. From there, your complaint will be investigated by the appropriate Manager and Director. You will receive a written response within 5 work days. The CEO will be kept informed about both the complaint and the proposed resolution.
Please note: CMHA Middlesex has a duty to report unacceptable, abusive and/or criminal behaviour to appropriate authorities. Complaints are reported regularly to the CEO, and annually to the Board of Directors as part of the Risk and Safety Report.
