We welcome all forms of feedback, both positive and negative, regarding services you have received at CMHA Middlesex.
CMHA Middlesex takes your concerns, complaints and feedback seriously. The feedback or complaint may be about the services you have received, or about an individual who provided you care. When we hear your feedback or complaint, it helps us continuously improve our services and our organization.
In terms of a complaint, the first step is to speak directly to the individual or service you are unhappy with. You may want to invite the Manager of that department. If the problem is still not resolved, please complete the Feedback Form below. From there, the appropriate Manager and Director will investigate your feedback or complaint. You will receive a written response within five workdays. The CEO will be kept informed about both the complaint and the proposed resolution.
Please note: CMHA Middlesex has a duty to report unacceptable, abusive and/or criminal behaviour to appropriate authorities. Complaints are reported regularly to the CEO, and annually to the Board of Directors as part of the Risk and Safety Report.
